- Information you type into the form: the type of plan you want, ZIP code and state, gender, date of birth, doctors, the name of the plan that is ending, your address, name, email, and phone number.
- Consent records: the exact consent language you agreed to, the time you agreed, your IP address, and your browser type.
- Basic usage data: the page you landed on, the site that referred you, and campaign tags in the link you clicked.
We do not collect Social Security numbers, Medicare numbers, bank details, or medical records through this website.
How we use it
- To have a licensed agent contact you about Medicare plan options, which is the reason you gave us the information.
- To check plan networks against your doctors and plan availability in your county.
- To keep records that federal and state insurance rules require us to keep.
- To understand which pages and campaigns bring people to the site.
Who we share it with
Your information goes to M&D Financial Agency. We share it with another licensed insurance agency only if you check the sharing box on the form and select that agency, and only with the agencies you select. Those agencies are listed on our Partners page. We also use service providers that host this website, store data, and send email on our behalf; they may process your information only to provide those services. We do not sell your information to data brokers and we never send marketing mail on behalf of unrelated businesses.
Calls and text messages
By submitting the form you agree that M&D Financial Agency and any partner you selected may call or text you at the number you provided, including with automated dialing technology or prerecorded messages. Consent is not a condition of purchase. You can withdraw consent at any time by replying STOP to a text, telling the agent who calls you, or calling (315) 992-1822.
